Pervasive Developmental Disorder

My son has had a tick since he was a baby (now 5) when he got excited he
would hold his breathe and wave both of his arms, wrists and fingers. He
still does it while playing with Hot Wheels, he set them up, take a step
back and open his mouth while shaking and twisting his arms. He will also
rub his eyes violently when he is going to sleep. when asked to stop doing
that, he comments, "well Dad, when I rub my eyes I can see a movie."  Other
than the tick he is an intelligent, and normal little boy.

Should we take him to see a neurologist?

That sounds less like a tick and more something on the PDD (Pervasive
Developmental Disorder, aka Autism) spectrum.  Either that, or they are just
idiosyncratic behaviors that he will grow out of.  The self-stim points to
something a bit more complicated, though.  If you're asking, you probably
want to have it looked at.
